It’s no secret that diabetes is on the rise

Primary care providers are on the decline and organizations are under pressure to do more—with less. Visits are getting shorter. Burnout is at an all time high.

To further complicate matters—diabetes management is complex. New medications, technologies, and clinical care guidelines often leave patients in therapeutic inertia. Care providers need more support.

Sarah Hormachea: Diabetes Care and Education was founded on the idea that while a robust care team would be amazing, it’s rarely financially feasible. Instead, I am committed to supporting providers when and where they need it most. I believe everyone deserves access to diabetes care and education and I’m willing to break the traditional healthcare model to turn that belief into a reality.
